Being developed to reduce body weight in adults with obesity or overweight with comorbidities, with or without type 2 diabetes; to improve glycaemic control in type 2 diabetes; and to reduce liver fat and treat MASLD and MASH. Boehringer emphasises a claimed preference for visceral and liver fat loss over lean mass, reporting a 34% visceral-fat and 63% liver-fat reduction in a phase 3 analysis.
It is not approved anywhere for any indication and has not been filed for obesity. Boehringer’s own material states: "Survodutide has not been approved for use, and its efficacy and safety have not been established." Its FDA Fast Track and Breakthrough Therapy designations, EMA PRIME acceptance and Chinese and Taiwanese breakthrough designations are all for MASH, not obesity — and a designation is not an approval.
Tolerability is a signal here rather than a footnote: in SYNCHRONIZE-2, 18% of survodutide recipients discontinued because of gastrointestinal adverse events against 1.2% on placebo, a rate publicly questioned at ADA 2026. The placebo arms also lost an unusually large amount of weight in these trials, which compresses the net effect, and the company releases quoted the more favourable efficacy estimand rather than the primary analysis.
Survodutide is sold online by "research peptide" vendors. Those products are not the studied product, have no verified identity, purity, potency or sterility testing and no regulatory oversight, and the trial data below do not describe them.
At a glance
A peptide that agonises both the GLP-1 receptor and the glucagon receptor. GLP-1 agonism reduces appetite and food intake; glucagon-receptor agonism is intended to increase energy expenditure and drive hepatic fat mobilisation, which is the stated rationale for the liver-directed programme. Unlike mazdutide it is a glucagon analogue rather than an oxyntomodulin analogue. The liver-fat result is the most direct human support for the glucagon arm: in SYNCHRONIZE-MASLD, 84.2% of recipients reached a 30% or greater relative liver-fat reduction against 24.3% on placebo. The relative contribution of each receptor to the weight effect has not been isolated in a human dose-dissection study, so the dual-agonist rationale remains a rationale.
Dated determinations, each flagged for whether it is final and whether it covers safety only. A safety review is not an efficacy finding, and most reporting in this category conflates the two.
Boehringer Ingelheim’s own material, including its release of 1 October 2026, states explicitly: "Survodutide has not been approved for use, and its efficacy and safety have not been established." No obesity marketing application has been publicly announced in any jurisdiction. Phase 3 obesity trials have read out (SYNCHRONIZE-1, SYNCHRONIZE-MASLD, SYNCHRONIZE-2) while SYNCHRONIZE-CVOT, SYNCHRONIZE-JP, SYNCHRONIZE-CN, the phase 3b trials SYNCHRONIZE-HERA, ELEVATE-LIVER and SYNCHRONIZE-START, and the MASH programme LIVERAGE and LIVERAGE-Cirrhosis are still running or starting.
Granted in September 2024, for MASH. Survodutide is not approved in Taiwan for any indication.
Granted by China’s National Medical Products Administration in June 2024, for MASH. Survodutide is not approved in China for any indication.
PRIME acceptance in November 2023, for MASH rather than obesity. PRIME is enhanced regulatory support for a promising development programme, not a determination that the medicine is effective or safe.
Fast Track designation in May 2021 and Breakthrough Therapy designation in September 2024. Both are for MASH, not obesity, and neither is an approval or any finding that the drug works. These designations are frequently reported as though they were regulatory endorsements of the obesity programme; they are not.
