Being developed as an amylin-based monotherapy for chronic weight management in adults with overweight or obesity, with the specific positioning claim that it delivers double-digit weight loss with gastrointestinal tolerability close to placebo — that is, as an option for people who cannot tolerate GLP-1-based drugs. Phase 2 also reported reductions in waist circumference, hsCRP and triglycerides.
It is not approved anywhere, and it is the earliest-stage compound here alongside amycretin. It has only just entered phase 3: the registrational ZUPREME programme of roughly 7,000 participants was announced as initiated on 22 September 2026, about three weeks before this research was compiled. No phase 3 results exist, no marketing application has been filed in any jurisdiction, and none can be for years.
The tolerability claim rests on a single 485-person phase 2 trial with no active comparator, sponsored by Roche. At the maximally effective dose that trial recorded no vomiting cases and no discontinuations for gastrointestinal adverse events; that signal is real and it has not been reproduced at scale. There is no data beyond about 51 weeks, no cardiovascular outcome data and no marketing experience of any kind.
Petrelintide is sold online by "research peptide" vendors. Those products are not the studied product and none of the data below transfers to them.
At a glance
An analogue of amylin, a hormone normally co-secreted with insulin by pancreatic beta cells after eating. It agonises amylin receptors, promoting satiation through a pathway separate from the incretin system. Roche’s stated rationale is that amylin-receptor activation reduces body weight partly by restoring sensitivity to leptin, so that fullness is reached sooner. Those two halves have different standing. That amylin-receptor agonism produces weight loss in humans is directly demonstrated by the placebo-controlled ZUPREME-1 result. **The leptin-resensitisation explanation is a preclinical hypothesis**: no human data isolating it were found. The proximate mechanism is human-confirmed; the story told about why it works is not.
Dated determinations, each flagged for whether it is final and whether it covers safety only. A safety review is not an efficacy finding, and most reporting in this category conflates the two.
Petrelintide had only just entered phase 3 when this research was compiled. The registrational phase 3a ZUPREME programme was announced as initiated on 22 September 2026 — ZUPREME-3 (about 3,900 participants, obesity or overweight with a comorbidity, no diabetes), ZUPREME-4 (about 600, with type 2 diabetes) and ZUPREME-5 (about 2,500, with established cardiovascular disease), roughly 7,000 participants in total, all with a primary endpoint of percent change in body weight to week 64, and ZUPREME-5 running up to three years. No phase 3 results exist, no application has been filed anywhere, and none can be for years. A phase 2 combination trial with Roche’s enicepatide was planned for the second half of 2026.
