The one compound in this cluster that is a legally available approved medicine in a Western market. Approved in the United States on 1 April 2026 as Foundayo for adults with obesity, or overweight plus at least one weight-related condition, alongside a reduced-calorie diet and increased physical activity, and authorised in the United Kingdom on 10 August 2026 for weight management and, additionally there, to improve glycaemic control in type 2 diabetes. It is prescription-only in both.
It is not approved for type 2 diabetes in the United States. Lilly also reports reductions in waist circumference, non-HDL cholesterol, triglycerides and systolic blood pressure; those are surrogate risk markers, not demonstrated outcome benefits, and no completed outcome trial shows a reduction in cardiovascular events.
Its controlled dataset is the largest here, and it was generated entirely by its manufacturer: ATTAIN-1 and ATTAIN-2 were designed, funded and analysed by Eli Lilly, with Lilly employees among the authors. The longest controlled exposure is 72 weeks and post-marketing experience was about six months old in October 2026, so long-term safety is not yet established.
Compounded or "research-grade" versions sold outside the regulated supply chain are not the approved product, and the data below do not describe them.
At a glance
A small non-peptide molecule that binds and activates the GLP-1 receptor, described as a partial agonist, reproducing the main pharmacology of endogenous GLP-1: reduced food intake, delayed gastric emptying and glucose-dependent insulin secretion. Because it is not a peptide it is orally bioavailable without a permeation enhancer and without food or water restrictions, which distinguishes it from oral semaglutide. This is the one mechanism in the cluster that is confirmed in humans rather than inferred. GLP-1 receptor agonism and its downstream effects are extensively demonstrated both for this molecule, from phase 1 through phase 3, and for the drug class as a whole.
Dated determinations, each flagged for whether it is final and whether it covers safety only. A safety review is not an efficacy finding, and most reporting in this category conflates the two.
Authorised 10 August 2026 for weight loss and weight maintenance in adults with BMI 30 or above, or BMI 27 to 30 with at least one weight-related comorbidity, and additionally to improve glycaemic control in insufficiently controlled type 2 diabetes. Prescription-only. Further phase 3 readouts — the ACHIEVE diabetes programme, the ATTAIN switch studies and ACHIEVE-4, a cardiovascular safety trial — continued through 2026, so the dossier is still growing after approval.
Approved 1 April 2026 for adults with obesity, or overweight plus at least one weight-related comorbid condition, alongside a reduced-calorie diet and increased physical activity. The FDA described it as the first new molecular entity approved under its National Priority Voucher Program. Prescription-only.
The United States approval covers obesity and overweight only. A United States type 2 diabetes submission was planned or filed during 2026 and no diabetes approval had occurred as of 13 October 2026. The same molecule carries a diabetes indication in the United Kingdom and not in the United States, so an orforglipron claim is only as good as the jurisdiction it is made in.
