Four separate measurements, never averaged. Independent evidence counts only studies with no sponsor attached. Methodology version v2.0-2026-10 · how we grade evidence
The first GLP-1 agonist approved for weight management, and now off patent. Approved for adults, adolescents from 12, and in the European Union for children from the age of six. Separately approved for glycaemic control in type 2 diabetes and, in people with type 2 diabetes at high cardiovascular risk, for reducing major adverse cardiovascular events.
The gap between its sponsored and independent evidence is the widest in this cluster. Its own pivotal trial found a 5.4 percentage-point advantage over placebo. In the 2026 independent network meta-analysis it was not convincingly different from lifestyle modification alone on body weight at one year, while carrying one of the highest discontinuation rates in the class. Both results are shown here because both are real.
Unlike semaglutide and tirzepatide, it showed little or no lean-mass loss against lifestyle alone.
At a glance
A GLP-1 analogue with a fatty-acid side chain that binds albumin, extending its half-life enough for once-daily use. Receptor agonism produces glucose-dependent insulin secretion, glucagon suppression, slowed gastric emptying and central appetite suppression. Reduced energy intake, increased satiety and delayed gastric emptying are all directly demonstrated in humans.
Dated determinations, each flagged for whether it is final and whether it covers safety only. A safety review is not an efficacy finding, and most reporting in this category conflates the two.
The indication now covers adults with BMI 30 or above (or 27 to under 30 with a weight-related comorbidity), adolescents from 12 with obesity and body weight above 60 kg, and children aged 6 to under 12 with obesity and body weight of at least 45 kg — the widest paediatric reach of any drug in this cluster. Generic liraglutide has since entered the market.
An FDA advisory committee voted 14 to 1 in favour on 11 September 2014; approval followed on 23 December 2014. Extended to adolescents aged 12 to 17 on 4 December 2020. Victoza, for type 2 diabetes, was approved on or before 25 January 2010.
