Claimed to suppress appetite and produce weight loss by expanding in the stomach, lower cholesterol, improve blood glucose and relieve constipation. The weight claim fails on the evidence while being authorised in law. A random-effects meta-analysis of eight randomised trials in overweight or obese participants found body weight −0.22 kg (95% CI −0.62 to 0.19), I² = 65% — not statistically significant, a confidence interval that comfortably includes zero and slight weight gain, and a point estimate inside the range of normal day-to-day body-weight fluctuation. The European Union nevertheless authorises a weight-loss claim for glucomannan, one of very few such claims it grants.
The cholesterol effect holds up better but on a thin base: 12 studies with only 370 participants in total gave LDL cholesterol −0.35 mmol/L (95% CI −0.46 to −0.25), about a 10% relative reduction, and non-HDL cholesterol −0.32 mmol/L. Apolipoprotein B did not move in the six trials that measured it, and since apoB is the particle-count measure, its being unmoved while LDL-C falls weakens the atherogenic interpretation.
Safety knowledge is moderate. The gel-forming property that supplies the mechanism is also the hazard: choking and oesophageal and intestinal obstruction, which is the explicit basis for the warning the European Union mandates on every authorised claim. Recorded adverse events were abdominal discomfort, diarrhoea and constipation. No hepatotoxicity signal; long-term safety data are limited and the included trials were short.
At a glance
The most viscous of the common soluble fibres. It forms a bulky gel proposed to delay gastric emptying and increase satiety, and it binds bile acids, driving hepatic cholesterol conversion and LDL-receptor upregulation. The mechanism is confirmed only in part: the bile-acid and cholesterol pathway is replicated with tight confidence intervals, while the satiety-to-weight-loss chain fails at the final step. Appetite may be affected; body weight, in pooled randomised data, is not.
Dated determinations, each flagged for whether it is final and whether it covers safety only. A safety review is not an efficacy finding, and most reporting in this category conflates the two.
FDA-specific action on glucomannan was not verified in this pass and is not asserted.
In the Annex to Regulation (EU) 432/2012, usable only for food providing a daily intake of 4 g of glucomannan, with labelling stating the effect is obtained at that intake. This is the better-supported of the two authorised claims: LDL cholesterol −0.35 mmol/L, although from only 370 pooled participants and with apolipoprotein B unmoved.
In the Annex to Regulation (EU) 432/2012, usable only for food containing 1 g of glucomannan per quantified portion, with labelling stating a daily intake of 3 g in three 1 g doses with 1–2 glasses of water before meals, within an energy-restricted diet. This is the most instructive regulatory anomaly in the dataset: one of very few authorised weight-loss claims in the European Union, attached to a compound whose pooled randomised weight effect is −0.22 kg (95% CI −0.62 to 0.19), not statistically significant.
Both Annex entries carry a warning for people with swallowing difficulties or inadequate fluid intake, with advice to take with plenty of water. The gel-forming property that supplies the mechanism is also the hazard: choking and oesophageal and intestinal obstruction. Konjac-jelly confectionery has been subject to choking-related recalls, not verified in detail in this pass.
